NEW CHOICES WAIVER PROGRAM

Allowing more control of your life

This program is designed for individuals ages 18 or older that are residing in a long term care facility. It gives them the option to move into a community-based setting if they wish to do so.

Qualifications for Program:

  • Must be at least 18 years or older

  • Must meet Utah Medicaid financial eligibility requirements 

  • Must live in a nursing home for at least 90 days or reside in an assisted living facility on an extended stay basis of at least 365 days or more

Services Provided: 

  • Adult companion services

  • Case management

  • Chore services

  • Emergency response systems

  • Fiscal management services

  • Home delivered supplemental meals

  • Homemaker services

  • Medication reminder systems

  • Non-Medical transportation

  • Personal attendant program training

  • Personal attendant services

  • Personal budget assistance

  • Personal care assistance

  • Respite care assistance

  • Respite care services (may be provided in home or long-term care setting)

  • Specialized medical equipment

 You are able to choose your own case management agency and we would love to serve you. Unfortunately, because we are a case management agency, we are not able to help with the application process. Instead, you will need to contact the state office at 1-800-662-9651 and select option 6.
